The documents that are required along with application form and valid original Passport are:

1. Bonefide Certificate/Work Letter/ HR Letter (depending upon the profession of the applicant)
2. A) Valid Chamber of Commerce of NTN certificate of the employing company (if the applicant is sole proprietor/partner of employed in corporate sector).
However: if employed in any multinational/international company then there is no need to attach NTN/Membership certificate.
B) If applicant is Doctor and runs his own clinic then registration certificate from PMDC is required. 
C) If applicant is Lawyer then certificates from Bar Council is required.

6. A) Personal Bank Statement for the last 03 months ( company’s bank statement can be attached in case if the company will bear the travelling expenses; however this should be mentioned in the work letter of the applicant.
B) If the applicant is minor/unemployed and he/she does not have own bank statement then bank statement of any guardian/parents can be attached along with the affidavit (where there is to be mentioned that expenses would be borne by the person whose bank statement is attached).
